8.   VOID CHECK OR BANK PRE-AUTHORIZATION FORM
9.   IN BANKRUPTCY OR ORDERLY PAYMENT OF DEBT...WE REQUIRE LETTER FROM TRUSTTE STATING       YOUR  ACCOUNT IS IN GOOD STANDING .
